Procedure for Applying
- Initial Contact: Call 702-689-5710 to express interest in the Medicaid-Personal Care Attendant program.
- Assessment Appointment: Schedule an appointment for an initial assessment to determine eligibility and care requirements.
- Medicaid Approval: Await Medicaid’s approval for the assigned weekly hours based on the assessment.
- Transfer of Services: If the client is currently with another company but wishes to switch, we assist in transferring services seamlessly to our company.
- Service Agreement: Establish a service agreement outlining the terms of service and care plan.
- Begin Services: Once approved, personalized care services commence as per the agreed care plan.

- Proof of Medicaid eligibility
- Personal identification (e.g., driver’s license, ID card)
- Medical records or doctor’s note specifying the need for attendant care (if applicable)
- Any other documents requested during the assessment
