  Services can be contracted with or without Medicaid coverage.

4. Service Eligibility

To access our services, the client must:

• Be a resident of Las Vegas, NV, or surrounding areas.
• Meet the eligibility requirements set by Medicaid (if applicable).
• Provide accurate medical and contact information.

5. Client Rights and Responsibilities

The client must:

• Allow access to the caregiver for service provision.
• Provide a safe environment for the caregiver.
• Inform of any changes in health condition or needs.
• Comply with payment and cancellation conditions.

6. ADA Personal Care LLC Rights and Responsibilities

We commit to:

• Providing services with professionalism and respect.
• Protecting the privacy and security of clients.
• Assigning trained and responsible caregivers.
  We reserve the right to modify services or decline clients under justified circumstances.

7. Liability Limitations

ADA Personal Care LLC will not be responsible for:

• Indirect or incidental damages resulting from the use of our services.
• Accidents or situations beyond our control.

9. Cancellations and Refunds

• Prior notification is required for cancellations.
